Onsite Check-in

Coach/Manager or Team Official must have the following documents ready for pre-tournament check-in at the Midland Soccer Complex registration tent if not completed online prior to MIT.

Check-in/packet pickup will open Friday evening (May 6th) from 4 to 8 pm and Saturday morning (May 8th) from 7 am to noon. 

  • League roster for current season (Fall 2021/Spring 2022) signed by league official

  • Player pass cards (electronic pass cards will not be accepted)

  • Guest Player Form (if applicable) To be completed on MSYSA website under Tournament Center.

  • Risk Management card (or equivalent) for all coaches, assistant coaches and Team Managers

  • Coach/Assistant Coach CDC Heads-Up Concussion Certification

  • Out-of-state teams must provide a copy of their permission to travel form.

MSYSA policies indicate that as a sanctioned tournament- participating teams are responsible for presenting the appropriate documentation to the proper forms relating to concussions and medical releases for each individual player if asked. Make sure that these are in your possession at the tournament but they do not need to be checked in by MIT officials. (i.e., Player signed Michigan Department of Community Health/CDC Parent and Athlete Concussion Information Sheet, etc.)
